Case Management & Aftercare

Treatment is the beginning of a lifelong journey of discovery and recovery, but what happens after treatment is equally as important as the treatment itself.

We at The Oaks at La Paloma make every effort to assure that each client has a workable continuing care plan and the structure to sustain the growth generated in treatment.

Before clients leave, we make sure each individual is equipped for what comes next. Clients are given:

  • Follow-up appointments with therapists and physicians
  • Contacts for 12-Step programs
  • Other support contacts

Contact with the many friends that were made while in treatment is also maintained through the alumni program and through HeroesInRecovery.com, a nationwide recovery community.

Intensive Outpatient Program

This can include the Intensive Outpatient (IOP) program, which can be invaluable as the client transitions back to “real life” or for past clients who have relapsed or feel they are in danger of relapsing.

Aftercare Group

Another great option is The Oaks at La Paloma’s Aftercare group, which meets weekly and is free to all alumni. The continuing support of this peer-led group provides a safe place to discuss the ongoing issues that arise during long-term recovery.

Sober Living

Treatment doesn’t end when clients leave The Oaks at La Paloma. Recovery is a lifelong process and we’re committed to your long-term success. For some clients that includes the support of a Sober Living program.

Approximately 15 percent of clients that complete our program will transition to a Sober Living environment, providing extra support with more freedom so they can put into practice the skills they’ve learned in a structured environment. For other clients, an intensive outpatient program is recommended.

No matter which option is the right fit, the The Oaks at La Paloma staff works with each client to find a program near their home and will even check with their private insurance to find the most cost-effective option.
